The VA Community Care Network is a great option for in-home care for veterans of all ages who qualify. This applies to those with both service-connected and non-service-connected disabilities.
Through the VA CCN Program, veterans can choose any credentialed agency that provides in-home care to receive care.
The VA usually assigns veterans between 6 to 20 hours of care per week. In some cases, however, the VA may authorize care for 24 hours a day. The VA bases the number of hours a veteran receives solely on need, not financial standing. The VA reimburses the VA CCN providers directly, NO OUT-OF-POCKET expense! Some veterans can be eligible for both in-home care services through the VA CCN and the Aid and Attendance Pension benefit.

Key Steps to Access VA CCN In-Home Care Services
1. Enrollment in the VA Health Care System
Application: Veterans need to apply for enrollment in the VA health care system using the 10-10EZ form. This form is the starting point for applying for VA health benefits, including geriatrics and extended care.
Submission: Submit the form online, in person, by mail, or by telephone to a VA health care facility.
Processing: Processing takes approximately one week. After that, veterans will receive an approval letter and a call to help establish their care.
2. Estabilishing Care with the VA
First Appointment: The VA assigns veterans a primary care physician (PCP). They will meet the clinical team, also known as the Patient Aligned Care Team (PACT). The Patient Aligned Care Team includes a doctor, social worker, and nurse.
Advocacy: Veterans should clearly express their need for home care to ensure appropriate support levels.
3. Eligibility Assessment by VCCP
Evaluation: The Veterans Community Care Program (VCCP) will determine if the veteran's disability is service-connected. The program will also determine if the veteran qualifies for community-based services.
Categories: Veterans are eligible for community care regardless of whether their disability is service-connected. Enrolled veterans are eligible for various VA programs and services.
4. Order for HHA Services
Evaluation: The Veterans Community Care Program (VCCP) will determine if the veteran's disability is service-connected. The program will also determine if the veteran qualifies for community-based services.
Categories: Veterans are eligible for community care regardless of whether their disability is service-connected. Enrolled veterans are eligible for various VA programs and services.
5. Coordination and Provision of Services
Authorization: The HHA Program Support Assistant will place the veteran with a credentialed HHA provider (Caring Companions😊) Then, the HHA Program Support Assistant will authorize the provision of services.
Scheduling: The selected provider will coordinate with the veteran to schedule the in-home care services.
